With PE-backed groups and DSOs offering competitive salaries, it's a fair question: why take on the risk of ownership at all? The answer is in the math, not the lifestyle.
An employed physician trades time for a salary, full stop. An owner builds an asset that appreciates, generates cash flow beyond their own labor, and can eventually be sold — often for more than they paid, especially with the tax advantages of an installment sale on exit. Over a 10–15 year horizon, the wealth gap between an employed physician and a practice owner in the same specialty is typically substantial, even after accounting for the additional risk and administrative burden of ownership.
The biggest barrier to ownership has never been clinical readiness — it's been capital and deal complexity. Both have gotten meaningfully easier.
Seller financing means you don't need $1M+ in cash or a perfect credit profile to acquire a practice — a well-qualified physician buyer with 10–20% down and a clean license can often structure a deal with the seller carrying the rest. SBA 7(a) loans are increasingly tailored to healthcare acquisitions specifically, with lenders who understand practice cash flow rather than treating it like a generic small business loan. And retiring physicians — a wave of whom are exiting over the next decade — are actively looking for buyers who'll preserve what they built, not just the highest bidder.
